Caretakers
Caretakers need support too. Caretakers are also the survivor's second line of communication when you are unreachable. All information are private only to Aortic Warriors and our administrators.

Please enter AGE at the time of first incident.
Please enter DATE(s) of your rupture / dissection.
Please enter how it was initially managed. ie. Immediate surgery, inoperable, managed by blood pressure medication, etc.
Please enter any changes of aortic health management.
Why you should register and what Aortic Warriors do with these information
The only reason why Aortic Warriors collects these information is so our survivors will never feel alone. It allows us to follow up with your progress. It allows us to get in contact with your Caretaker in case we don't hear from you. The list will never be shared or sold.
